What information does Auckland Sleep collect about you?

When you enquire about our services or when you become a patient of Auckland Sleep or otherwise use our services, a record is made which includes your personal information. The type of personal information that we collect will vary depending on the circumstances of collection and the kind of service that you request from us, but will typically include:

  • your full name
  • your gender
  • your contact details (including your residential address, phone number and email address)
  • your date of birth
  • your relevant health care or insurance agency related identifiers, such as your, NHI number. This is to support payment or referral processes if this applies to your care
  • sensitive information (including details about your health and clinical history in relation to your sleep disorder)


How does Auckland Sleep collect your personal information?

We generally collect personal information directly from you when you register or make a booking with us, subscribe to receiving emails from us, complete an online survey or visit our clinic. We may collect and update your personal information over the phone, by email, over the internet or social media, or in person.

We may also collect personal information about you from other sources, for example:

  • authorised persons or persons who act on your behalf
  • other health service providers (for example, we may receive referrals from your family doctor that contain your personal information)


For what purposes does Auckland Sleep collect, hold, use and disclose your personal information?

The purposes for which Auckland Sleep usually collects, holds, uses and discloses personal information depends on the nature of your interaction with us. This may include:

  • verifying your identity
  • providing services and products to you (such as to help you, as a patient, monitor the progress of your treatment plans in communicating with various sleep consultants, to support you in the management of your sleep issue.)
  • informing you about our health services and products that may be of interest to you
  • fulfilling administrative, management and operational functions associated with our services and products (such as administering billing and payments, quality improvement activities)
  • researching, developing and innovating our products and services
  • responding to enquiries and complaints


How does Auckland Sleep hold information?

Auckland Sleep has systems and procedures in place to protect your information from misuse and loss, unauthorised access, modification or disclosure.

Your information is stored securely and is accessed only by staff providing services to you, or in the course of carrying out our services.

Our staff all sign confidentiality agreements when they join us, and there are systems and policies in place to prevent them sharing data inappropriately.

If you want to see a copy of your information

If you want to check information that we hold about you, please see below for contact details. We will ask you to provide evidence that you are in fact the person whose information you are requesting, or that you are entitled to this information.

We will not charge you a fee for requesting a copy of your information unless we have already provided you with the same or substantially the same information within the last 12 months.

If you believe any information we hold about you is incorrect you can ask us to correct it. We will either make the correction or explain to you why we are not prepared to do so. In that case you may ask for a statement of your views to be placed on your record.
